The patient who is coming to AIMS first time for the treatment shall be registered at AIMS. Patient/relative is requested to furnish full and correct demographic details at the time of admission only. The registration form shall be filled up and signed by the patient or patient relative only. Upon the registration, the patient is given a unique IP No & an identity card of the hospital.
Patient would also be given an ID band also which shall be worn by the patient all the time during the stay. You will also be provided the admission kit on admission with all toiletries like soap, tooth brush, tooth paste, shampoo etc. If in case patient requires any changes in the information after registration, valid proof for the amendment shall be produced.
